What You’ll Be Doing
As part of our innovative hospice and palliative care team, you’ll play a crucial role in providing compassionate support to patients and families facing life-limiting illnesses. Your responsibilities will include:
- Delivering psychosocial support and social work services to hospice patients and their families in various settings, from hospitals to home care.
- Collaborating with the hospice interdisciplinary group (IDG) and serving as a vital link between hospice staff and inpatient or nursing home teams.
- Creating tailored social work plans and goals based on the unique needs of each patient and their family.
- Developing discharge plans when needed for patients in inpatient facilities.
What You’ll Need to Succeed
To thrive in this role, you should have:
- A degree from an accredited school of social work
- At least one year of experience in a healthcare setting
- Familiarity with Microsoft Word and Excel (knowledge preferred, but not mandatory)
- A valid New York State MSW license (required)
- Preferably a New York State driver’s license
- The ability to thrive in a team environment
- Top-notch organizational, communication, and listening skills
- Proficiency in inputting clinical data
- Fluency in English and Chinese (Mandarin or Cantonese)
Compensation
- Minimum Salary: $64,000/year
- Maximum Salary: $80,000/year
