Position Overview
In this role, you’ll be at the heart of creating personalized care plans for our clients and members. You’ll assess their needs and coordinate services with various healthcare providers to support individuals facing complex psychiatric issues, substance use challenges, and co-existing medical conditions. Your goal is to ensure they have access to a comprehensive range of physical and mental health services, as well as community resources to tackle social determinants of health. You’ll facilitate smooth communication among providers, always keeping the best interest of your clients at the forefront as you work under general supervision.

What You’ll Do
- Conduct assessments to pinpoint the needs of clients/members and craft both initial and ongoing care plans.
- Regularly update plans based on any changes in the client’s status or circumstances.
- Manage a caseload effectively, tracking progress and ensuring timely and accurate documentation.
- Create a resource inventory tailored to the needs identified during assessments.
- Connect clients/members with the appropriate service providers and managed care plans, facilitating case record reviews and conferences.
- Work closely with team members to engage clients who may be hesitant to accept services.
- Advocate for clients/members and educate them and their families on available entitlements and community services, including arranging transportation to appointments when needed.
- Attend ongoing training to continuously enhance your care management skills.
- Maintain thorough and updated case records in the program’s EMR system, adhering to VNS Health’s standards.
- Collaborate in case conferences and staff meetings to assess whether clients/members need different levels of care or are ready for discharge.
Qualifications
Education:
- A Bachelor’s Degree in a human services-related field is required.
- Enrollment or attendance in a Master’s degree program in a relevant field is preferred.
Experience:
- A minimum of two years of hands-on experience providing services to clients with Serious Mental Illness (SMI), developmental disabilities, substance use disorders, or chronic medical conditions is essential if you hold a Bachelor’s. If you have a Master’s degree, at least one year of relevant experience is required.
- Strong verbal and written communication skills are a must.
- Bilingual abilities may be required depending on operational needs.
- Basic computer skills are necessary.
Compensation
Hourly pay ranges from $23.17 to $28.96.
